BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2016-217518 DESCRIPTION An elderly patient was admitted for a coronary artery bypass graft. The postoperative course was complicated by decompensated atrial fibrillation requiring repeat endotracheal intubation. On examination subcutaneous emphysema was noted, which progressed to involve the upper arms, cervical region and face. Chest X-ray and CT scan demonstrated subcutaneous emphysema and pneumomediastinum (figures 1 and 2). Fibre optic bronchoscopy revealed a normal tracheobronchial tree, without obvious orifices in the tracheal wall. A CT scan of the thorax revealed a tracheal diverticulum on the right posterolateral aspect of the trachea (figures 3–5). A defect in the superoanterior wall of the diverticulum was identified as the site of rupture. Rupture was likely secondary to postintubation barotrauma. Given the absence

Infected tracheal diverticulum: a rare association with alpha-1 antitrypsin deficiency* **
Tracheal diverticulum, defined as a benign outpouching of the tracheal wall, is rarely diagnosed in clinical practice. It can be congenital or acquired in origin, and most cases are asymptomatic, typically being diagnosed postmortem. We report a case of a 69-year-old woman who was hospitalized after presenting with fever, fatigue, pleuritic chest pain, and a right neck mass complicated by dysph...
متن کاملManagement of A Patient with Kommerrell's Aneurysm Causing Tracheal and Esophageal Compression
SUMMARY Tracheal and esophageal compression is a well-recognized complication of aneurysms of the aortic arch. Most of the patients present with dysphagia and/or respiratory insufficiency. In the adult population a right-sided aortic arch is often asymptomatic unless aneurysmal disease develops. This usually occurs at the level of the take-off of an aberrant left subclavian artery and is known ...
متن کاملTracheal rupture resulting in life-threatening subcutaneous emphysema.
We present a case of a patient with severe chronic obstructive pulmonary disease who developed dramatic mediastinal and subcutaneous emphysema, without pneumothorax, following a difficult intubation. Misdiagnosis of tracheal rupture as barotrauma from alveolar overdistention initially delayed intervention and caused persistence of subcutaneous emphysema. Despite efforts to minimize tidal volume...
متن کاملTracheal Diverticulum: A Unique Case with Intraoperative Morphologic Assessment
There are rare case reports of tracheal diverticula or paratracheal air cysts. These cases, however, were reported mostly as incidental sonographic or radiologic findings without histologic confirmation. Furthermore, the handful of studies that describe this entity histopathologically report only cases in patients with prior respiratory symptoms. Here, we report a rare case of an asymptomatic 6...
متن کاملVesicoenteric fistula caused by bladder diverticulitis: MRI Findings.
The images presented below were from a man, at the age of 80, who was admitted to hospital with recurrent urinary tract infection. Magnetic resonance imaging (MRI) was performed after initial examination. Conventional MRI showed multiple diverticulum in bladder and one of them had contact sigmoid colon via fistula tract (Figure-1). Diffusion weighted images (DWI) showed restricted diffusion in ...
